CARE Program
CARE –
Comprehensive Assistance with
Recovery and Empowerment
The CARE program is designed as a fluent service for persons, who have met the SMI criteria, to enter and exit the program as needed.
Persons may remain in the program for an exteneded period of time.
A core clinical assessment is completed upon admission. This allows persons to begin the process of joining with their treament team to determine
how they best see their recovery journey unfolding. CARE services recognizes that persons entering services have unique strenths and challenges.
Therefore, CARE services are designed to encourage persons to build up their physical, emotional and intellectual skills. These skills are intertwined
with persons developing happy, healthy and productive lives in their community environment.
Therapeutic interventions are individually structured to emphasize person's development, enhancement and acquisition in skill building in the areas of
personal, social and emotional growth. Co-occurring services are provided concurrently with mental health services. Therapeutic models that work with
Evidence Based Practiced are embraced in CARE services.
These include but are not limited to:
1.) Dialectical Behavioral Therapy
2.) Acceptance and Commitment Therapy
3.) Developmental Psychology
4.) Persons Centered Therapy
5.) Cognitive/Behavioral Therapy
We recognize that other therapeutic modalities may also be utilized to assure that persons recieve the interventions that best fit their needs to
help them achieve their recovery goals.
The CARE team is made up of 10 case managers, a vocational specialist, a clinical director and an administrative director. The team is multi-disciplinary
to best meet the needs of the persons served. CARE services are provided 24 hours a day, 7 days a week, always with someone available to handle
emergencies.
