JOB TITLE: Psychiatric Nurse – IMPACT Team

REPORTS TO: Program Manager & Director of Nursing

Salary:  DOEE $45,000 -$50,000/year

SUMMARY The job of psychiatric nurse is a clinical, direct service position.  The job consists of psychiatric nursing and case management services to Community Counseling Services’ clients that are served through a distinct program – IMPACT. The IMPACT program serves adult individuals identified as having severe and persistent mental illnesses and their families.

 EXPECTATIONS Community Counseling Services expects employees to be welcoming to consumers, including those with co-occurring disorders, and engaging them at their current stage of recovery. Services provided will be in accordance with the plans and guidelines as established by Community Counseling Services and the State Division of Behavioral Health.  Portray and encourage civility to all clients, customers, employees, community partners and vendors.  

QUALIFICATIONS Education and Experience

  • Possession of a minimum of an LPN, bachelor’s or master’s degree in nursing or other health-related field is highly desirable.

Certificates, licenses, registrations

  • Minimum of LPN, RN preferred.
  • Valid South Dakota Driver’s license.
  • Maintain licensure status.
  • Skills, knowledge, and/or experience that demonstrates an ability to deal appropriately and in a caring manner with individuals with severe and persistent illnesses.
  • Must not be excluded from participating in Medicare, Medicaid, and any Federal health care program, as provided by the OIG, USDHHS.

  • Must have a valid driver’s license or the ability to obtain one and be

willing to transport consumers     ESSENTIAL DUTIES/CRITICAL JOB ELEMENTS Nursing Direct all nursing activities with the IMPACT team in providing medication administration and medical services, as directed by the Nursing Director and Medical Director.

  1. Give injections as prescribed
  2. Utilize E-Scribing System
  3. Assist Prescribers as needed.
  4. Coordination of State and indigent funding applications for medication
  5. Will lead in medication, pharmacy and other medical service activities
  6. Coordination of required Lab Work
  7. Coordination of schedule of medical assessments of client physical health, making appropriate referrals to physicians and psychiatric treatment with medical treatment.
  8. Provide crisis intervention on call services for medical questions from the team. Backup would be Director of Nursing

  Case Management   Provide case management services to consumers and families as defined by the Division of Behavioral Health and Community Counseling Services and will include: Client Care  

  1. Individual, group skills building with clients, liaison services with referral sources and family members.
  2. Develop and update treatment plans in a timely manner
  3. Documentation of all client related contacts reflecting individual treatment plan
  4. Complete progress reports and discharge summaries in a timely manner
  5. Participate in and utilize treatment teams concepts
  6. Participate in staff meetings
  7. Complete outcome tool as directed by Clinical Director
  8. Maintain knowledge of IMPACT of contract with the SD Division of Behavioral Health and ACT standards, follow ACT fidelity scales with SD specific modifications
  9. Provide psychiatric rehabilitation services. Support clients in building skills for independent living in the community, including, but not limited to, shopping, cooking, cleaning, personal hygiene, budgeting, socialization, and accessing transportation
  10. Other duties as assigned
  11. A full time provider will work 40 hours a week.

Licensure & Credentialing

  • Maintain professional licensure
  • Participate with credentialing staff to establish and maintain billing/admitting/service credentials
  • If eligible, gain QMHP status.
