SUMMARY

The job of case manager is a clinical, direct service position. The job consists of case management services by meeting with individuals, or having collateral contact with care givers, that only receive psychiatric services with our prescribing physicians at CCS to formulate treatment plans twice yearly.

 

EXPECTATIONS

Community Counseling Services expects employees to be welcoming to consumers, including those with co-occurring disorders, and engaging them at their current stage of recovery. Services provided will be in accordance with the plans and guidelines as established by Community Counseling Services and the State Division of Behavioral Health.  Portray and encourage civility to all clients, customers, employees, community partners and vendors.

 

QUALIFICATIONS

Education and Experience

  • The Case Manager shall have achieved at least an associate’s degree, bachelor’s degree preferred, in the social sciences. Experience working with children, families, and/or vulnerable population is also preferred.
  • Skills, knowledge, and/or experience that demonstrates an ability to deal appropriately and in a caring manner with individuals with severe mental illness.
  • Basic knowledge regarding developmental stages and needs of children.

Certificates, licenses, registrations

  • Driver’s license
  • Must not be excluded from participating in Medicare, Medicaid, and any Federal health care program, as provided by the OIG, USDHHS.

Other qualifications

  • Ability to work effectively in a multi-disciplinary environment.
  • A strong commitment to working as an integral part of a team.
  • Must have a valid driver’s license or the ability to obtain one and be

willing to transport consumers

 

 

ESSENTIAL DUTIES/CRITICAL JOB ELEMENTS

 

Case Management

 

Provide case management services to consumers and families as defined by the Division of Behavioral Health and Community Counseling Services and will include:

Client Care

  1. Develop and update treatment plans in a timely manner.
  2. Documentation of all clients related contacts reflecting individual treatment plan.
  3. Complete progress reports in a timely manner.
  4. Participate in and utilize treatment team concepts.
  5. Case management and liaison services with family and referral sources.
  6. Participate in weekly staff meetings;
  7. Provide on-call crisis intervention on a rotating schedule to include evenings, weekends, and holidays.
  8. Complete Division required surveys as directed by Clinical Supervisor
  9. Other duties as assigned.
  10. A full time provider will work 40 hours a week.
  11. Due to the nature of the position, the provider will be expected to work a minimum of one evening per week if needed.
