SUMMARY
The job of SOC is a clinical, direct service position.  The SOC portion of this position services children/families of non CCS clients that have been identified and referred by their primary school as needed extra supports to be successful in the school and home environment.  This position links children and families to appropriate community supports.

The Systems of Care (SOC) program is a wraparound approach to delivering services to at-risk youth and families, as identified by school systems and other community stakeholders.  The Coordinator services children/families of non CCS clients that have been identified and referred by their primary school as needed extra supports to be successful in the school and home environment.

 

EXPECTATIONS

Community Counseling Services expects employees to be welcoming to consumers, including those with co-occurring disorders, and engaging them at their current stage of recovery. Services provided will be in accordance with the plans and guidelines as established by Community Counseling Services and the State Division of Behavioral Health.  Portray and encourage civility to all clients, customers, employees, community partners and vendors.

• Must not be excluded from participating in Medicare, Medicaid, and any Federal health care program, as provided by the OIG, USDHHS.

• Must have a valid driver’s license or the ability to obtain one and be
willing to transport consumers

ESSENTIAL DUTIES/CRITICAL JOB ELEMENTS

  1. Provide treatment services, as required by the Division of Behavioral Health, shall utilize evidence-based practices focusing on juvenile risk factors to improve mental health, and to reduce the likelihood of delinquent behavior.
  2. Employ the SOC values of being strengths-based and family-driven as well as culturally competent.
  3. Provide families information about and methods of accessing resources and are able to determine what services will best meet their needs.
  4. Provide for the use of natural resources as well as Family Support Program funds to assist with resources that are otherwise unavailable in the community.
  5. Work in collaboration with many agencies including school systems and other community resources, to provide the following services:
    1. Consultation with schools and community agencies
    2. Accept referrals and ensuring completion of intake paperwork
    3. Assess and assist families with needs across life domains
    4. Ensure families are informed of service options
    5. Facilitate team meetings as needed to develop and monitor service plans and ensure that service plans are driven by the family
    6. Provide case management to refer and facilitate access to an array of community services and support
  6. Provide collaboration and coordination to facilitate implementation of service plans across all involved child and family serving agencies
  7. Continually update knowledge of and relationship with community resources available to children and families
  8. Conducting training, outreach and marketing in support of SOC programming
  9. The care coordinator position should be based out of the local school system or other neutral location.
  10. The care coordinator should maintain a caseload of, on average, no more than 20 active families.
  11. Agency agrees to ensure care coordinator is trained in Systems of Care and has or will receive trainings regarding cultural competency and trauma informed care within one year of hire.
  12. Agency agrees to work jointly with the Division of Behavioral Health, local school administrators, and other community stakeholders to design and implement Systems of Care programming at a local level including regularly scheduled meetings to review services and outcomes, ensure collaborative communication, and conduct problem solving.
  13. A full time provider will work 40 hours a week.
  14. Due to the nature of the position, the provider will be expected to work a minimum of one evening per week if needed
  15. Other duties as assigned.
