JOB TITLE: HEALTH HOME COORDINATOR

REPORTS TO:  NURSING DIRECTOR

SUMMARY

The job of the Health Home Coordinator is a clinical, direct service position.  The job consists of assessing and coordinating individualized care plans developed by the designated provider with active participation of all health care team members.

EXPECTATIONS

Community Counseling Services expects employees to be welcoming to consumers, including those with co-occurring disorders, and engaging them at their current stage of recovery. Services provided will be in accordance with the plans and guidelines as established by Community Counseling Services and the State Division of Behavioral Health.  Portray and encourage civility to all clients, customers, employees, community partners and vendors.

QUALIFICATIONS

Education and Experience

  • Possession of a minimum of an medical assistant (associates degree), LPN, bachelor’s or master’s degree in nursing or minimum of associates degree in other behavioral health-related field is highly desirable.

Certificates, licenses, registrations

  • Minimum of Medical Assistant, LPN, RN preferred.
  • Valid South Dakota Driver’s license.
  • Maintain licensure status. 
  • Skills, knowledge, and/or experience that demonstrates an ability to deal appropriately and in a caring manner with individuals with severe and persistent illnesses.
  • Must not be excluded from participating in Medicare, Medicaid, and any Federal health care program, as provided by the OIG, USDHHS.

Other qualifications

  • Ability to work effectively in a multi-disciplinary environment.
  • A strong commitment to working as an integral part of a team.
  • Must have a valid driver’s license or the ability to obtain one and be                

            willing to transport consumers in agency vehicle.

ESSENTIAL DUTIES/CRITICAL JOB ELEMENTS

Client Care

  1. Conduct client intake, assessments and diagnostic interviews
  2. Develop and update treatment plans in a timely manner.
  3. Documentation of all clients related contacts reflecting individual treatment plan.
  4. Complete progress reports and discharge summaries in a timely manner.
  5. Referral to appropriate services to meet client needs.
  6. Participate in and utilize treatment teams concepts.
  7. Case management and liaison services with family and referral sources.
  8. Participate in staff meetings as needed;
  9. Manage Health Home Case Load
  10. Complete outcome tool as directed by Health Home Program – quarterly reports
  11. Assist all nursing activities within the agency, in providing medication administration and medical services, as directed by the Nursing Director and Medical Director.
    1. Give injections as prescribed
    2. Maintain and organize Medication Administration Records (MARS)
    3. Utilize E-Scribing System
    4. Assist Prescribers as needed.
    5. Implement Meaningful Use objectives as directed by Nursing Director
    6. Assist with State and indigent funding applications for medication
  12. Assist with new staff orientation
  13. Other duties as assigned.
  14. A full time provider will work 40 hours a week.

 Licensure & Credentialing

  1.  Maintain professional licensure
  2. Participate with credentialing staff to establish and maintain billing/admitting/service credentials
