JOB DESCRIPTION

JOB TITLE: Nurse/MEDICAL ASSISTANT

LOCATION: MADISON

REPORTS TO: CLINICAL SUPERVISOR & NURSING DIRECTOR

FLSA  STATUS: NONEXEMPT

DEVELOPED: 6-19

SUMMARY

The job of Nurse/medical assistant is a clinical, direct service position.  The job consists of Nursing/medical assistant services to Community Counseling Services consumers. Work week is 4 days (M-Th).

EXPECTATIONS

Community Counseling Services expects employees to be welcoming to consumers, including those with co-occurring disorders, and engaging them at their current stage of recovery. Services provided will be in accordance with the plans and guidelines as established by Community Counseling Services and the State Division of Behavioral Health.  Portray and encourage civility to all clients, customers, employees, community partners and vendors.

QUALIFICATIONS

Education and Experience

  • The Case Manager shall have achieved at least an associate’s degree, bachelor’s degree preferred, in the social sciences or medical field.  Experience working with children, families, and/or vulnerable populations is also preferred. 
  • Skills, knowledge, and/or experience that demonstrates an ability to deal appropriately and in a caring manner with individuals with severe and persistent illnesses.
  • Basic knowledge regarding developmental stages and needs of children.

Certificates, licenses, registrations

  • Driver’s license
  • Must not be excluded from participating in Medicare, Medicaid, and any Federal health care program, as provided by the OIG, USDHHS.

Other qualifications

  • Ability to work effectively in a multi-disciplinary environment.
  • A strong commitment to working as an integral part of a team.
  • Must have a valid driver’s license or the ability to obtain one and be willing to transport consumers       

 

ESSENTIAL DUTIES/CRITICAL JOB ELEMENTS

 

Provide services to consumers and families as defined by the Division of Behavioral Health and Community Counseling Services and will include:

MEDICAL DUTIES

 Direct all nursing activities with the outpatient team in providing medication administration and medical services, as directed by the Nursing Director and Medical Director.

Maintain and organize Medication Administration Records (MARS)

Utilize E-Scribing System

Assist Prescribers as needed.

Assist with State and indigent funding applications for medication

Complete a medication administration-training course to qualify for assisting with administration of prescription and nonprescription medications. The training will be in accordance with guidelines as defined in the Division of Behavioral Health contract.

Other duties as assigned.

A full time provider will work 40 hours a week.

Client Care

 

Conduct client intake, assessments and diagnostic interviews

Individual and group skills building with clients and family members.

Develop and update treatment plans in a timely manner.

Documentation of all clients related contacts reflecting individual treatment plan.

Complete progress reports and discharge summaries in a timely manner.

Referral to appropriate services to meet client needs.

Participate in and utilize treatment teams concepts.

Case management and liaison services with family and referral sources.

Participate in staff meetings;

Complete outcome tool as directed by Clinical Supervisor

Complete Division required surveys as directed by Clinical Supervisor

Provide ongoing assessment, problems solving, skills training, supervision and environmental adaptations to assist the consumer in performing basic daily living activities including (but not limited to):

Medical, dental and vision services coordination ;

Linking to social service agencies, Legal advocacy and representation;

Provide medication, nutrition education,

Educating consumers and families regarding mental illness and recovery;

Teaching and role modeling appropriate social skills and emotion regulation in a variety of situations;

Consult with the family to ensure quality of care, problem solve and mediation;

Interpersonal relationship encouragement, support, mediation and role-modeling healthy relationships;

Collaboration with substance abuse services as appropriate
