Community Counseling Services’ SED program is similar to the CARE program but for children. The children’s program is intensive, child-centered, and family-focused. Services are individualized in accordance with the unique needs of children.

CARE services recognize that persons entering services have unique strengths and challenges. Therefore, CARE services are designed to encourage persons to build up their physical, emotional, and intellectual skills. These skills are intertwined with people developing happy, healthy, and productive lives in their community environment.

SOC program is a wraparound approach to delivering services to at-risk youth and families, as identified by the school systems and other community stakeholders.  A youth is available for SOC services up to the age of 21 and must have identified needs in at least one of the following domains; basic needs, social supports, emotional needs, educational needs, community supports, housing supports, health, or safety.  Only one client per family may be served at the time.  SOC Case Managers consult with schools and community agencies, assess and assist families with needs across life domains, and collaborate and coordinate to facilitate the implementation of an identified service.

Individualized and Mobile Program of Assertive Community Treatment (IMPACT) helps consumers take care of their basic needs, including getting up, taking their medications, and sometimes just making it through the day. IMPACT works with customers to help them decide which medications best meet their needs. They also help consumers find housing, get food stamps, go back to school, or get a job. IMPACT helps consumers struggling with mental illness to get back on their feet.

The Bradfield-Leary Center Apartments provide supportive housing to our consumers who participate in the CARE or IMPACT Programs. We offer efficiency units on the 2nd floor with a community kitchen and 1 bedroom apartments on the 3rd floor. Laundry facilities are provided on both floors.

Community Counseling Services Psychiatric department has several years of medication management experience.  These services are provided via telemedicine, by one of our Psychiatrists, or by one of our Nurse Practitioners.  Our prescribers are assisted by our efficient nursing team, who help answer medication questions and complete refills.  Telemedicine services can be accessed by presenting at our office in Huron, our office in Madison, or in the privacy of your own home.  We are professional, compassionate, and realize that every client is unique.  We offer individualized treatment plans in an outpatient setting to improve you or your loved one’s quality of life.
